Testicular germ cell tumours (TGCTs) are the most common solid malignancy in young men, and with cure rates exceeding 95%, long-term survivorship, including fatherhood, has become a central goal of care. Many patients already have impaired spermatogenesis at diagnosis, and orchiectomy, platinum-based chemotherapy, radiotherapy and retroperitoneal lymph node dissection may further compromise fertility, ejaculatory and endocrine function. This webinar will review current evidence on semen quality and hormonal status at diagnosis, the dose-dependent effects of chemotherapy and radiotherapy on spermatogenesis and sperm DNA integrity, and the expected time course of recovery. Recent findings on sperm functional characteristics after chemotherapy will also be highlighted.
